What are the four steps in the incident investigation process?

Introduction Of  Incident :

The incident could be defined as the result of an unplanned, undesired and uncontrollable event. An incident is a mother of all events, may include ill health, dangerous occurrence, near-miss of damage to property.

Injury Accident

It could be defined as the result o an unplanned, undesired and uncontrollable event that causes the worker to suffer physical injury. 

Damage Only Accident 

It could be defined as the result of an unplanned, undesired and uncontrollable event that results in significant damage to machinery, materials properties.

ill-Health 

Term ill-health could be defined as it is a harm to a work in health because of their work and include physical or physiological harm.

Near Miss

It could be defined as the result of an unplanned, undesired and uncontrollable event that did not cause the injury, equipment damage etc. But note the harm could have done in somewhat different circumstances.

Dangerous Occurrence

It could be defined as a very significant hazardous incident with a severe outcome or with the potential to cause severe consequences in the future.

• E.g overturning of a mobile crane 
• Leakage of LPG gas from filling station.
